(HHI) is a new nonprofit organization dedicated to providing access to quality, affordable health care services for uninsured adult residents of Howard County, Maryland. HHIs Healthy Howard Health Plan is one of two local plans in the country to offer a network of health care services to uninsured residents and the first in the nation to couple services with required health coaching to improve overall health outcomes. Health Coach Responsibilities
Each Plan member is matched with a Health Coach to help him/her develop a health action plan and assist in identifying and eliminating barriers to achieving health goals. Coaching activities will follow a defined protocol but will be tailored to the individual member and involve a mix of in-person and telephone contacts. Each full-time Health Coach will be responsible for maintaining a caseload of up to 150 members and documenting all member interactions per HHI protocol. As a part of the team headed by the Lead Health Coach, Health Coaches will combine training in the evidence-based HHI Health Coaching Model with their own professional knowledge and experiences to provide quality, compassionate guidance and support that honors the autonomy of Plan members. After the initial orientation period, Health Coaches work as telecommuters and are required to travel throughout Howard County, in addition to attending regular administrative meetings at the HHI offices.
Job Summary
-Maintain caseload of members and perform Health Coach duties in accordance with program policies and procedures
-Coordinate with other HHI departments and community resources to meet member needs and resolve issues.
-Produce required reports and participate in regular staff meetings to share ideas, discuss concerns, receive ongoing training, and improve service delivery
-Share ideas and challenges with peer Health Coaches and the Lead Health Coach to improve program and member outcomes.
-Able to work flexible hours, including occasional evenings and weekends
-Participate in HHI outreach and enrollment activities as needed.
Requirements
-Minimum Bachelors degree in health education, health sciences, nursing, nutrition, or related health discipline.
-3 + years experience working in health and lifestyle management with individuals or groups in a community setting, health care facility, or related organization.
-Excellent communication and interpersonal skills.
-Knowledge of behavior change and health management processes.
-Demonstrated experience in motivating and supporting individuals, particularly in goal setting activities. Prior experience with diverse client population preferred.
-Attention to detail and strong organizational skills.
-Bilingual language skills (Spanish/English or Korean/English) highly desirable.
